For too long, youth mental health systems have measured referrals instead of care. A young person in distress is screened, referred, and then disappears from the record. The connected care pathway changes that by making follow-through visible at every step.
What the pathway does
The pathway connects three layers: a low-pressure youth check-in (Snuggli), a care coordination workflow for frontline teams (Sentra), and a measurement layer that tracks whether care actually happened. Each step is logged, auditable, and owned — so a referral that goes unanswered is seen and followed up, not silently dropped.
Built for where care actually happens
The infrastructure is designed offline-first for the low-connectivity realities of most of the world, and it keeps clinical boundaries human-led: AI may support reflection, documentation, and routing, but it cannot diagnose, prescribe, or close a case.
